How do I find a doctor in the network?

DocFirst does not limit your access. You can see any doctor you and your family doctor select as long as you follow the guidelines.

How do I know if my family doctor or specialist is "In-Network"?
DocFirst does not limit your access, meaning there are no physician networks. You and your family doctor pick the specialty provider that is right for you.

Is an obstetrician/gynecologist considered a family doctor? What if I am currently pregnant?
No, an obstetrician/gynecologist is considered a specialist. If you are currently seeking treatment from an obstetrician/gynecologist, you will need to contact your family doctor and complete a Consult Verification Form to receive the highest level of benefits.

What about my annual OB/GYN visit?
Generally a Consult Verification Form is not requiresd for your annual OB/GYN visit.

My child is away at school, who is considered the family doctor?
If your child is away and enrolled in school or a university, DocFirst considers the physicians practicing at student health care center a family physician. DocFirst must have each of these physicians listed on the Family Doctor Identification Form. After the physicians are identified, they can fill out the Consult Verification Form for any specialty care needed. This form is valid for one year, unless an expiration date is listed.

When should I notify DocFirst if I become pregnant?
You should notify DocFirst in the first trimester (the first three months) of your pregnancy.

Can I only see a specialist if I am referred by my family doctor?
You can see any specialist of your choice but you must first speak to your family doctor and have him or her complete the Consult Verification Form. Often your family doctor will know who the best specialists for your condition are in your geographic area. You can seek a specialist without consulting your family doctor cost will be higher.

Do I have to get a referral if my specialist sends me for tests?
As long as you have a Consult Verification Form to see your specialist, an additional verification form is not needed for tests requested by the specialist.

Can I get a 2nd opinion?
You can seek a second opinion as long as you and your family doctor complete the Consult Verification Form.

My doctor is not familiar with DocFirst, and will not accept the copayment only, what should I do?
Call our Customer Care Center at 1-888-883-4458 or e-mail us at customercare@lbax.com with your doctor’s name, address and telephone number. DocFirst will contact your doctor to explain the program and attempt to reach agreement regarding the copay and billing procedures. This does not guarantee your doctor will accept only the copayment. In these instances you must pay the doctor and submit the claim to DocFirst. We will process it promptly.
